Community Health & Hospice, Inc. is a private, non-profit home health and community support service agency serving the greater Lakes Region of New Hampshire.  Our four programs (Young Family Program, Home Care, Support Services, and Hospice) help strengthen and support families from birth through bereavement. Our staff of 125 delivers approximately 50,000 home visits each year in the Lakes Region and travels nearly a quarter of a million miles annually.

The organization was founded in 1975 as a visiting nurse organization known as Lakes Region Community Health Agency.  In 1992, the VNA merged with a non-profit hospice organization, Hospice - Laconia Area, Inc. to form Community Health & Hospice, Inc.   The agency is governed by a 12 to 15 member, voluntary board of directors drawn from the communities we serve.

Community Health & Hospice operates a single office site, located in Laconia.  The agency is licensed by the state of New Hampshire and certified by Medicare for both the home care and hospice programs.  During FY 2001, the agency achieved voluntary accreditation through the Community Health Accreditation Program (CHAP) - which it continues to maintain. In addition, the agency partners with other organizations to implement several programs, thus avoiding duplication.  Some of these partnerships include Lakes Region General Hospital,  Belknap/Merrimack Community Action Program (Head Start), and ServiceLink of Belknap County.  

The agency also participates as a funded partner with the Lakes Region United Way (www.lruw.org).
